In short
An affidavit or statutory declaration is a written statement of fact affirmed before an authorised officer. In Thailand the affirmation is normally taken by a Notarial Services Attorney; for use abroad the document then follows the Apostille or legalisation chain required by the destination.
Drafting discipline matters: state facts within your own knowledge, identify each exhibit clearly, and avoid argument. Institutions abroad reject declarations that read as advocacy rather than as evidence.
Some destinations prescribe the exact wording of the jurat or the declaration clause. Where the receiving office has a template, use it verbatim — a substituted form is a common reason for a document to be sent back after it has already been legalised.

Watch out
Sign only in front of the officer. A pre-signed declaration cannot be affirmed retrospectively.
